How to File a Complaint

- Clearly detail your complaint, providing as much relevant information and evidence as possible.
- Submit Your Complaint:
- Use the online complaint form or submit your complaint via email or post to the designated department or individual.
- Track and Follow Up:
- Keep a record of your complaint submission and follow up as needed during the review process.

These resources adhere to the California Code of Regulations, Title 5 (5 CCR) sections 4600–4694, and are applicable to various programs and activities subject to the UCP.
Williams Complaint Issues
