Important Notice Regarding Form 1095-C (Employer-Provided Health Insurance Offer and Coverage):
Under US Treasury Regulation 1.6055-1(g)(4)(ii)(B), employers are no longer required to furnish annual health coverage statements (Form 1095-C) automatically to covered individuals; rather, these forms may be made available upon request. Beginning with calendar year 2025, if an employee requires a Form 1095-C, they can request one as follows:
